As a Home Care Support Worker, you will provide practical, social and personal support to clients in their homes, helping them maintain independence, dignity and quality of life.

· Provide personal care including showering, dressing, grooming and toileting
· Assist with domestic support such as cleaning, laundry and meal preparation
· Support community access including appointments, shopping and social outings
· Deliver companionship and emotional support
· Promote independence, choice and dignity
· Observe and report changes in client wellbeing
· Maintain accurate documentation and communication with the care team
