EACH is seeking a Care Coordinator for its Care Finder Program across Outer South Victoria with flexible full or part time and hybrid work. The role requires travel through Cranbourne, Narre Warren, Clyde, Pakenham, Carrum Downs, Officer and surrounding Casey, Cardinia and Frankston areas. Salary starts from $46.70 per hour plus super and salary packaging up to $18,550. You will engage older Australians facing vulnerability, use assertive outreach and a person centred approach to help them access aged care and community supports, guide My Aged Care processes, build referrals and maintain case notes. Qualifications in Social Work or similar and a valid driver’s licence are required; second language is a plus. The role
Here at Each we are looking for passionate Care Coordinators to join our care finder program, supporting older people who may be experiencing vulnerability, isolation or difficulty navigating the aged care system.
This role is all about meeting people where they are, building trust, and ensuring they don’t fall through the gaps when accessing vital supports.
As a Care Coordinator, you will use assertive outreach and a person‑centred approach to help older Australians understand, access and navigate aged care and other community services. Your day may include:
