Ennoble Care is seeking a full-time Care Coordinator for an office-based role in downtown Tulsa to support patient-centered care through EMR documentation, follow-up tracking, and timely provider communication within a family-practice style team. Ideal candidates have at least one year of clinical or care coordination experience and hold an active credential such as PCT, CNA, MA/CMA, LPN, or RN, with proficiency in Microsoft 365. You will coordinate transitions, referrals, and maintain accurate documentation to enable prompt decision making. This is a Monday through Friday, 8:30 a.m. to 5:00 p.m. role with no weekends or on-call, plus a comprehensive benefits package. Ennoble Care is seeking a full-time Care Coordinator to join our team in an office-based setting in downtown Tulsa, Oklahoma. This role is an integral, in-office member of our clinical care team, working alongside fellow Care Coordinators and collaborating with Ennoble Care providers within a family-practice–style model of care.
While not patient-facing, the position plays a critical role in supporting patient-centered care coordination, documentation, and communication, helping ensure continuity of care and timely follow-up across care settings.
