Medibank is recruiting a Care Integration Nurse or Paramedic for a part-time 0.6 FTE role in Adelaide, delivering patient-centred care through the Amplar Health Home Hospital model with home visits from our Eastwood office using a company vehicle. You’ll collaborate with Medical Officers, Nurse Care Coordinators, our provider network and LHNs, performing clinical reviews and home visits. Essential: Bachelor of Nursing or Paramedicine with unrestricted AHPRA, minimum 3 years’ experience, current Basic Life Support, and flexible 7-day roster (7am to 3pm or 2pm to 10pm). IV cannulation or willingness to gain, NSQHS knowledge, Working with Children Check, Police Clearance, and vaccinations. As a Care Integration Nurse or Paramedic, you will collaborate as part of a broader team working closely with Medical Officers, Nurse Care Coordinators, our service provider network and the Local Health Networks (LHNs).
You will be responsible for delivering patient-centred care, to suitable patients for Amplar Health Home Hospital (AHHH) and undertaking clinical reviews as part of a multi-disciplinary team.

This role will involve undertaking visits in the community to our patients’ homes delivering nursing care as required by their care plan. You will have use of our company vehicle for patient visits starting your shift from our office in Eastwood.
This position will be offered as a part time (0.6 FTE) position, with rostered hours across a 7-day roster across two shifts 7am to 3pm or 2pm to 10pm.
