Berry Street is hiring a Case Manager I for the Transitional Youth Support Services in Preston to provide housing focused case management, assertive outreach, and early intervention for young people aged 16 to 25 at risk of homelessness. In this full time, permanent role you’ll help secure safe housing, build independent living skills, and develop exit plans within a multidisciplinary TYSS team. Key requirements include a Bachelor in Social Work, Youth Work or related field, experience in homelessness services, trauma and family systems knowledge (including MARAM), and strong engagement, planning and advocacy skills. Berry Street’s Transitional Youth Support Services (TYSS) supports young people aged 16–25 who are experiencing homeless or at risk of homelessness through assertive outreach, housing support and early intervention.
Within TYSS, the Homelessness Intervention and Prevention (HIP) stream provides early intervention for young people aged 16–18 at risk of homelessness due to family conflict or breakdown.
As a Case Manager, you will deliver housing-focused case management, outreach and early intervention to support young people and families to achieve safe, stable housing, build independent living skills and strengthen connections to family, community and support networks.
