Clinical Coordinator Palliative Care

POSITION SUMMARY: 

The Clinical Coordinator of Palliative Care works under the direction of the Senior Director of Nursing and performs activities in accordance with the mission and vision of the organization and the philosophy of the department of nursing. The coordinator works collaboratively with other members of the interdisciplinary team; functioning as a registered nurse, team leader and central coordinator for clinical services rendered to our palliative care patients. The clinical coordinator is a patient and family advocate, facilitating services according to the patient’s wishes. The coordinator participates in patient’s rounds, leads weekly team meetings, works with the Medical Director and Attending Physicians on the Palliative Care Service Team to plan monthly program meetings and is available as a resource to staff. The Palliative Care Coordinator assumes an active role in the Graduate Medical Education Program. The coordinator is responsible for maintaining information pertinent for quality assurance purposes and program development. Promotes cooperation, a positive attitude, and creates a safe environment in which the patient and family are comfortable and supported in sharing information. Responsible for knowing and adhering to established hospital policies and procedures, including all aspects of safety.

RESPONSIBILITIES:

  • In collaboration with the team; performs comprehensive formal assessments of the referred patient and family, documenting reasons for admission, and reviewing problems and symptoms as a basis for developing a plan of care. 
  • Participates and may lead team and family conferences. 
  • Functions as a team leader and resource to other staff concerning palliative care. 
  • Communicates with social worker and case management in planning patient’s care. 
  • Reports and records patient’s symptoms, response to therapeutic measures and progress. 
  • Participates in daily patient rounds, as determined by the inter-disciplinary team.
  • Participates in the discharging planning and maintains an active knowledge of community resources. 
  • Chairs the Ethics Committee
  • Collects and reports Palliative care Data to SBUH for collaboration and alignment of Quality Initiatives. 

     

REQUIRED EXPERIENCE AND QUALIFICATIONS:

  • Current Licensure to practice in New York State. 
  • Bachelors in nursing. 
  • Experience in hospice/palliative care. 
  • Excellent communication Skills. 
  • Computer literate.

PREFERRED EXPERIENCE AND QUALIFICATIONS:

  • Master's degree in nursing

     

PHYSICAL DEMANDS:

  • Standing, walking 90% of the time. 
  • Lifting patients with assistance

     

As part of employment at Southampton Hospital, there may be occasions when travel to other locations is needed to support operational needs.

Anticipated Pay Range:

The starting salary range (or hiring range) for this position has been established based on relevant experience.

The above salary range (or hiring range) represents SBSH’s good faith and reasonable estimate of the range of possible compensation at the time of posting.

  • Meet Regulatory Requirements for pre-employment screenings.
  • Provide a copy of any required New York State license(s)/certificate(s).
