In this role you’ll deliver personal care, light housekeeping, and assistance with daily activities in participants’ homes and at the PACE center, following each person’s plan of care. A day in the life with Summit Eldercare
Under RN guidance, you assist with Activities of Daily Living for participants at the PACE center and in their homes, handling tasks such as transfers, bathing, dressing, toileting, and other routine cares as needed. You’ll help shape each participant’s plan by talking with the interdisciplinary team and ensuring care remains coordinated and appropriate. In addition to direct care, you’ll support activities staff in planning and delivering individual and group programs to motivate participation, and you may assist therapists with prescribed treatments and exercises, including positioning and ambulation.
Keeping a clean, safe environment is part of the job, whether you’re at the center or in a participant’s home, and you’ll help with meal preparation and feeding when needed. You’ll document accurately in the EMR and adhere to infection-control protocols, including regular cleaning and disinfection routines in line with current CDC guidelines. The role also involves contributing to the daily workflow across the site, helping to distribute tasks so that care needs are met in a timely way, and taking part in regular health aide team meetings to stay aligned with the broader plan of care.
Key prerequisites
