Plena Healthcare is seeking a Community Physiotherapist to join our Melbourne North West team during an exciting growth phase, in a full time mobile role supported by a large multidisciplinary network now part of Australian Unity. You will deliver evidence based, person centred care in clients’ homes, manage diverse cases, support activities of daily living, assess falls risk and equipment needs, and collaborate with colleagues across services. As a Community Physiotherapist, you will join a large, supportive multidisciplinary team delivering personalised, evidence-based care to help consumers maintain independence, function, and quality of life.
This is a full time (Monday - Friday) mobile community role in Melbourne's North Western suburbs.
We are open to early career or graduate therapists who are motivated to learn, enjoy variety, and are comfortable travelling between clients.
You will manage a varied caseload across acute and chronic conditions affecting movement, function, and independence.
