MacKillop Family Services is seeking a full-time Early Intervention Case Manager in Port Hedland WA to deliver initial screening and family case planning in the Pilbara. You will identify barriers, provide in-home support for carers and children, uphold a trauma-informed, family-centred approach, and coordinate with the Department of Communities and other providers while honouring Aboriginal culture. The package includes SCHCADS Level 4–5 pay, super, a $5,000 allowance, and salary packaging. About the role:
The EIS Case Manager will provide initial screening and assessment and develop with families, their family case plan. They will identify barriers and provide practical, in-home support for carers, oversee case management activity, provide parenting and trauma information for carers and specific supports for children to strengthen links to community and culture.
The EIS Case Manager will coordinate with and provide information to the coordinator for them to report to the Department of Communities and maintain positive relationships with key stakeholders relevant to the program, maintain a child-focussed, family centred approach and recognise the trauma that children and their extended families are likely to have experienced, be respectful and inclusive of Aboriginal culture through the informing of engagement strategies, assessment, and planning, use family, cultural mapping and genograms as an engagement strategy, in addition to being used as a safety and case planning tool and for building extended networks for families and provide ongoing liaison with the Department Case Manager and other service providers involved in supporting the family members through the regular review and report process
