MacKillop Family Services is hiring a full-time Home-Based Care Case Manager based in Dandenong South, VIC with remote work options. You will manage trauma-informed casework, supervise caregivers, complete LAC care planning, and guide transitions to permanent placements, reporting to the Home-Based Care Coordinator. Essential criteria include tertiary qualifications, strong casework skills with LAC and CIRSSP/CRIS, the ability to work collaboratively and independently, and a solid understanding of Aboriginal culture and current issues affecting Aboriginal children and families.
