You’ll conduct comprehensive assessments, manage acute and post-acute conditions, triage urgent situations, handle medication reconciliation, wound care, and documentation, delivering compassionate care in patients’ homes. Success depends on independent decision making, real time clinical judgment without on site support, strong problem solving for unpredictable home environments, and a flexible schedule with technology. Base Pay, depending on qualifications and subject to negotiations
$57.00 - $62.00 per point
