You’ll make home visits, collaborate with a multidisciplinary team, address social and emotional factors, provide clinical evaluations, coordinate community resources, advocate for clients and families, and document care on a tablet. The ideal candidate holds a MSW with at least 2 years of community-based experience and CSWE accreditation, can travel to cases, and has strong English communication and EMR skills; home care experience is a plus. Prior home care experience preferred, but not required.
