The ideal candidate will hold a Master’s in Social Work, CSWE accredited, with at least 2 years of community-based experience, strong English communication, and solid EMR skills. Key priorities include assessing social and emotional factors, coordinating community resources, advocating for clients, collaborating with a multidisciplinary team, documenting clearly, and traveling to cases. Travel from either:

Prior home care experience preferred, but not required. As a Medical Social Worker, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and palliative care to clients, affording them the opportunity to receive the care and support required to remain at home.

Base Pay, depending upon qualifications and subject to negotiation:
$36-$43 per hour
