Theoria Medical is seeking Advanced Practice Providers (Nurse Practitioners or Physician Assistants) for full time onsite roles in Paris, IL, delivering patient centered care in skilled nursing facilities with six hour shifts, no on call or overnight requirements, and off-site charting on an iPad LTE. You will round and write treatment plans, document in ChartEasy, and collaborate with facility staff while benefiting from technology such as an intuitive EHR, AI Scribe, and UpToDate. What You’ll Do

• Provide high-quality, patient-centered care to residents in skilled nursing facilities
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
• Maintain timely and accurate documentation in ChartEasy™
• Communicate with facility nursing staff via phone or secure messaging as needed
• Maintain reliable on-site attendance for scheduled shifts
• Ensure compliance with clinical standards, facility policies, and regulatory requirements

Your Qualifications

Advanced Practice Providers must hold an active U.S. license and be board certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include but not limited to Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
