About the Role

The primary responsibility of this role is to provide clinical care and professional advice and support as part of the Check-In Team based at Frontyard Youth Services ensuring delivery of an efficient, effective and clinically therapeutic service. 

Under the guidance of the Senior ,Occupational Therapist/Mental Health care coordinator ALERT Manager, Check-In Program Team Leader, and in conjunction with Frontyard team members, the Occupational Therapist/Mental Health care coordinator will provide and coordinate short to medium term specialist, targeted assessment and therapeutic interventions to consumers based at Frontyard.  

Interventions provided will be aimed at supporting psychosocial recovery, fostering individualised recovery, independence, and improved health outcomes for consumers with complex needs. Interventions and assessments provided could include, but not be limited to, function and capacity, sensory modulation, social skills, activity/vocation or additional treatment interventions to meet physical health or drug and/or alcohol needs. Interventions provided will either be on a 1:1 basis or delivered within a group. 

As a care coordinator, this role will also deliver holistic health assessments and facilitate client driven goal setting and care planning.. This role involves direct and indirect clinical care, maintaining an active caseload and supporting clients to develop self-management skills in relation to their needs. This position will receive clinical support from the Senior OT Mental Health Care Coordinator within the Check-In Program. Operational leadership onsite will sit with Frontyard. 


Classifications for this position will be Occupational Therapist Grade 2 VF6 - VF9 ($90,235 - $103,724 per annum, pro rata for part time).

Your Contribution

About Health Care for the Homeless

This position is part of the Health Care for the Homeless Program (HfH). HfH comprises several services that support clients that are experiencing or at risk of homelessness. HfH aims to integrate and better coordinate existing services within St Vincent’s Hospital and where service gaps are identified, support the development of new services to meet the needs of our clients.  

Operationally the role is based at Frontyard Youth Services which is a specialist youth service located in Melbourne’s CBD. Frontyard aims to support young people aged 12 to 25 to meet their physical, emotional and social needs and to develop pathways out of homelessness.  This includes but is not limited to providing 18 beds crisis accommodation service and drop in centre housing multiple services working with young people across Greater Melbourne and throughout Victoria.
