MacKillop Family Services seeks a full-time Out of Home Care Case Manager in West Perth to deliver quality case work for children in foster care, support foster carers and help recruit carers. You will manage a caseload, apply trauma-informed practice, conduct intake and assessment, support matching of care arrangements, and build links with the Department of Communities. The role requires a tertiary qualification in Social Work or equivalent, OOHC case work experience, knowledge of Aboriginal culture, strong planning and communication skills, and current checks including driver’s licence, WWCC and NDIS Worker Screening. About the role
The Case Manager is responsible for providing a high quality and effective case work service for children and young people in foster care. The role will also assist in the recruitment of foster carers, including providing day to day support and developing carers’ capacity, skills and understanding to best meet the needs of the children they are caring for.
The Case Manager will manage a case load of children and young people ensuring that case work practice, including documentation, is responsive to individual needs and reflects the organisation’s values, policies, practice frameworks in line Departmental expectations, ensure that children and young people in Out of Home Care receive responsive trauma informed care and targeted services that meet their needs, participate in intake and assessment of new referrals into the service, identifying the child or young person's individual needs and identifying capacity and making recommendations on suitable matches for care arrangements within the service’s current carer pool, ensure foster carers are supported to provide consistent high- quality care and positive outcomes for the children and young people in their care through regular carer support and development opportunities, establish and maintain collaborative relationships with the Department of Communities, and a wide range of specialist and mainstream services to improve outcomes for children, young people and their families.
This is a Full-Time opportunity based in West Perth, WA within our Out of Home Care, WA program. The position reports to the Out of Home Care Coordinator.
