This is a community-based Registered Nurse role focused on delivering high-quality, person-centred nursing care to older Australians living at home. You will work across both care management and office-based clinical functions, supporting customers with complex and escalated needs.
You’ll provide sound clinical assessment, care planning and intensive case management within your scope of practice, particularly for customers requiring high-level Support at Home, reablement, restorative and end-of-life care.
Working collaboratively within a multidisciplinary team, you will guide care planning, support service delivery teams with clinical expertise, and ensure customers receive timely, appropriate and evidence-based care.
