This part-time 0.4 FTE Youth Health Nurse role at EACH sits within the headspace Youth Health Clinic, working across DiSS sites and headspace Narre Warren & Dandenong on the lands of the Bunurong People, in a multidisciplinary team that supports young people’s physical, mental and sexual health. About the program
This role sits within the Youth Health Clinic at headspace, delivering care through the Doctors in Secondary Schools (DiSS) program and GP youth clinics. These services provide young people with free, accessible primary health care - addressing common barriers such as cost, transport, and appointment availability.

As part of a multidisciplinary team, the role works alongside GPs, nurses, school wellbeing teams, families, and headspace clinicians to support young people’s physical health, mental health, and sexual and reproductive health needs.

Your role, your impact
As a Youth Health Nurse, you’ll support young people to access timely, inclusive, and developmentally appropriate health care in environments where they already feel safe.

Your work will directly contribute to early intervention, improved health outcomes, and stronger connections between young people, families, schools, and community services.

You will:

• Provide clinical nursing care to young people within DiSS and headspace GP clinics
• Conduct assessments including HEADDSS, mental state and risk assessments
• Support care coordination and referrals with GPs and youth services
• Deliver health education and follow-up care for young people
• Collaborate with GPs, school staff, families and internal teams to deliver integrated care
• Maintain accurate clinical documentation and service data (including HAPI surveys)

About you
You’ll succeed in this role if you have:

• Registration as a Division 1 Nurse with AHPRA
• Experience working with young people in health or community settings
• Strong therapeutic engagement and communication skills
• Confidence assessing and responding to clinical risk
• Experience across physical health, mental health, sexual health and AOD
• Unrestricted Victorian driver’s licence
