Position Summary

The Clinical Manager is an office-based clinical leadership role within Assistive Care State Programs and is one of the core office roles identified for the practice. This role provides clinical oversight, case management support, caregiver supervision, skills development, and client-focused coordination to help clients remain safe at home with comfort, independence, and dignity.

Primary Responsibilities

  • Conduct regular skill-building during orientation and ongoing caregiver development, including coaching, training, and in-the-moment education during client visits.
  • Identify high-potential caregivers and partner with the office team to connect them to training, scholarships, and growth opportunities.
  • Maintain strong relationships with clients, families, caregivers, and office teammates through proactive communication, responsiveness, and follow-up.
  • Collaborate in matching clients and caregivers to support compatibility, continuity, and quality care.
  • Review and respond to client concerns, hospitalizations, schedule changes, and care issues on a regular basis.
  • Apply sound clinical judgment in complex cases and escalate matters appropriately to office or clinical leadership when needed.
  • Build and maintain a current community resource list with the office to help address needs beyond the care plan.
  • Lead difficult conversations professionally, using active listening, empathy, and solution-focused communication.
  • Support documentation accuracy, timely follow-up, and clear communication across caregivers, clients, families, and office partners.

Missouri State Programs Responsibilities

  • Directly observe caregivers providing care or service at least annually, or more frequently as required by regulation, program need, employee performance, or client need, and ensure supervision is documented appropriately.
  • Support ongoing employee compliance activities related to supervision, performance evaluations, required screenings, and annual education expectations.
  • In Missouri, support Advanced Personal Care (APC) training by partnering in skills demonstration, competency validation, and office lab training activities for designated caregivers.

Qualifications

  • RN License required in the state of MO.
  • Demonstrated ability to coach, develop, and supervise caregivers in home-based care settings.
  • Strong clinical judgment and the ability to manage complex cases, identify concerns, and escalate appropriately.
  • Effective communication and relationship-building skills with clients, families, caregivers, and interdisciplinary office partners.
  • Ability to organize work, manage changing priorities, and respond to client and office needs with adaptability.
  • Working knowledge of caregiver supervision, documentation expectations, compliance requirements, and client-centered service delivery in a state programs environment.

Work Environment

This role includes office-based responsibilities as well as home and community visits for client oversight, caregiver supervision, training, and competency support.
