POSITION SUMMARY:
Under the direction of the Program Coordinator, this position is responsible for helping to create effective links between appropriate health and social resources to minimize emergency room visits and hospital admissions; addressing social isolation and anchoring at risk individuals in the community. This position will adopt a holistic approach in helping clients to engage pro-actively with the community, to address social determinants of health and wellness. This position works in partnership with other stakeholders including hospital staff, primary care and other community organizations with a multi-disciplinary approach to facilitate clients transitioning from one environment to another to support living well in the community.
* This position requires access to a vehicle and a valid drivers license.
* This position will require occasional coverage in both Hamilton and Burlington areas*
EMPLOYMENT GUIDE:
