APM Group is hiring a Clinical Aged Care Assessor in Mackay for a full-time onsite role four days per week, with daily travel across the service area to assess needs, refer services, and help older Australians live independently while meeting Department of Health contractual requirements. You will conduct high quality assessments, promote functioning, engage families and carers, and collaborate in a multidisciplinary team with thorough training and flexible work options. Ideal candidates have at least one year working with older Australians, a health related degree with professional registration (eg AHPRA), strong Word, Excel and Outlook skills and client management systems, a current Australian driver’s license, comprehensively insured vehicle, and the ability to travel.
