IRT Group seeks a Home Care Partner to support aging in place in ACT and NSW with case management, referrals, assessments, goals and coordinated services. Requirements include a related qualification such as Cert IV in Community Services or Aged Care and a Case Management qualification with 1 to 2 years’ experience; excellent communication, attention to detail and data skills; dementia or mental health experience is a plus; driver’s licence and computer competence (Alaya Care a bonus). Preferred locations include ACT, Far South Coast, Illawarra, South East or South West Sydney; hybrid role with 3 days in-office and 2 at home. As a Home Care Partner, you will work closely with customers to enhance their quality of life, enabling them to remain at home while meeting their care needs.
You will provide case management for customers, fostering independence and promoting well-being – case management experience and qualifications are essential.
Our preference is for the successful candidate to be located in the ACT, Far South Coast, Illawarra region, South East or South West Sydney region. This is hybrid position – the expectation is to work from the nearest IRT office at least 3 days a week and 2 days working from home.
