An experienced RN Care Coordinator in Chronic Disease role at EACH in Campbelltown on the lands of the Dharawal People is a 0.6 FTE max term position until June 2027 with flexible days, Wednesday, Thursday and Friday preferred, and community outreach across South Western Sydney. Based in Campbelltown you will join the My Care Partners program, delivering holistic chronic disease management in the community, coordinating referrals, supporting self management and providing follow up after hospital discharge in collaboration with GPs and multidisciplinary teams. Requires Bachelor of Applied Science (Nursing) or equivalent, Division 1 RN registration with AHPRA, minimum Grade 2 Year 4 RN experience. About the program
My Care Partners is a primary care-led program supporting people living with chronic and complex health conditions to stay well at home and connected to the care they need. Working closely with general practices and community partners, the program focuses on coordinated, person-centred care that responds to the whole person—not just their diagnosis.

As a Registered Nurse Care Coordinator – Chronic Disease, you will play a pivotal role in supporting people to manage their health, build confidence, and navigate the health system with dignity and choice. Your work will help reduce preventable hospital admissions, strengthen continuity of care, and improve everyday wellbeing for people living with long-term health challenges.
