Who are we looking for?

We’re looking for a confident and experienced clinician with a minimum of two years working autonomously in the field to take on this role that will work with clients in the community while staying connected to a supportive, multidisciplinary team.

You will enjoy getting results for your clients as a result of your ability to conduct thorough assessments and applying sound clinical reasoning skills, which in turn will help older people maintain independence and quality of life at home.

You’re someone who:

  • Builds trust quickly with customers, families and stakeholders
  • Is confident working independently and making decisions in complex or changing situations
  • Has proven experience managing a varied case load of clients along with the associated workload
  • Communicates with clarity, empathy and professionalism

What can you expect to be doing?

Reporting to the Care Community Lead, this role will see you:

  • Conducting in-home assessments and reviews to guide care planning
  • Coordinating your own calendar and managing daily visits across the community
  • Supporting person-centered care focused on independence and re-enablement
  • Providing sound clinical advice and guidance on complex or escalated care needs
  • Collaborating with internal teams, providers and health professionals when required

What do you need to bring?

To thrive in this role, you will have:

  • Tertiary qualification in Nursing, Allied Health or related field
  • Current AHPRA registration (where applicable)
  • Experience in community aged care or complex care settings
  • Strong assessment, care planning and clinical decision-making skills
  • Ability to work autonomously and manage competing priorities

We would also love you to have:

  • Experience working within a wellness and re-enablement model
  • Confidence in case conferencing and multidisciplinary collaboration
  • Strong documentation and reporting skills
