AnglicareSA seeks an experienced clinician to join the Community Aged Care team as an Assessment Partner in Adelaide. The role requires a tertiary qualification in Nursing or Allied Health, AHPRA registration where applicable, and at least two years’ autonomous practice in community or complex care settings. You will conduct in-home assessments, guide care planning, manage a caseload, and provide clinical advice while collaborating with a health team to support independence and re-enablement. We’re looking for a confident and experienced clinician with a minimum of two years working autonomously in the field to take on this role that will work with clients in the community while staying connected to a supportive, multidisciplinary team.
You will enjoy getting results for your clients as a result of your ability to conduct thorough assessments and applying sound clinical reasoning skills, which in turn will help older people maintain independence and quality of life at home.
