As part of the interdisciplinary team, the RN Case Manager works under the direction of the Director of Nursing and in concert with the attending physician to deliver skilled nursing assessments, careful planning, and effective care. Care coordination in patient homes
You’ll oversee a defined patient caseload, guiding the nursing process from assessment through evaluation to ensure care plans align with patient goals and evolving needs. You’ll initiate and sustain contact with attending physicians, Hospice staff and external partners to coordinate resources and ensure optimal care for patients and their families, while keeping the physician informed about ongoing patient status. You’ll actively seek insights from other team members to enrich the plan of care and you’ll document accurately in the electronic medical records system. You’ll participate in interdisciplinary group meetings, contributing pertinent information that informs staff discussions, and you’ll help prepare caregivers and, when appropriate, provide direct care to support the patient’s needs. Regular collaboration with Hospice nursing staff helps address any caseload problems, with opportunities to share professional insights and promote growth. You’ll attend patient care conferences to discuss caseload issues with the broader team, supervise and document the NAC every two weeks or as assigned, and alert the Patient Care Coordinator to any unusual or potentially problematic situations. In times of off-hours coverage, you may step in as an associate nurse to support patient and family needs, and you’ll provide appropriate support at the time of death and during the bereavement period. The role also includes participating in orientation and ongoing in-service training, involvement in agency and community programs to deepen hospice understanding, staying up to date with agency policies and the rules governing hospice care, and taking on additional assignments as needed.

The starting pay for this full-time Hospice RN Case Manager is $85,000 per year.
