About the Role

An exciting opportunity exists for an experienced Grade 2 Social Worker to join our team on a permanent part-time basis.

Working within a supportive multidisciplinary environment, you will provide high-quality psychosocial assessment, intervention and discharge planning for patients within a sub-acute healthcare setting. This role offers the opportunity to work collaboratively with patients, families, carers and healthcare professionals to achieve positive patient outcomes and support safe transitions of care.

We are seeking a clinician with demonstrated experience in hospital social work who is passionate about person-centred care and committed to delivering evidence-based practice.

Experience in a sub-acute setting, particularly within Geriatric Evaluation and Management (GEM), will be highly regarded.

Classification: Grade 2 Social Worker (SC21 – SC24)
Salary: $90,235 – $103,724 per annum, pro rata

Key Responsibilities

  • Undertake comprehensive psychosocial assessments and formulate appropriate intervention and discharge plans.

  • Apply advanced clinical reasoning to support patients with complex psychosocial needs.

  • Liaise effectively with patients, families, carers and multidisciplinary team members to coordinate care and facilitate positive outcomes.

  • Support safe discharge planning and continuity of care.

  • Participate in quality improvement, service development and research activities.

  • Provide supervision and contribute to student and clinical education programs as required.

  • Foster strong working relationships within the department, across the organisation and with community-based services and stakeholders.

Your Contribution

To be successful in this role, you will have:

  • A Bachelor Degree or Qualifying Master's Degree in Social Work.

  • Significant social work experience within a hospital or related inpatient healthcare setting.

  • Demonstrated ability to undertake complex psychosocial assessments and interventions.

  • Strong communication, interpersonal and relationship-building skills.

  • Experience working collaboratively within multidisciplinary teams.

  • Current National Criminal History Check, or willingness to obtain.

  • Experience in sub-acute care and/or Geriatric Evaluation and Management (GEM) services will be highly regarded.
